2015 Medical Forms

In January 2015, all Forms A & B are due in, regardless of when in 2014 they were last submitted.

These forms should be downloaded online, filled out, and saved so you and your scout can sign, date (with a January date) and delivered to me or Scoutmaster Johnson to give to me. This medical information cannot be emailed, as nice as that would be, due to privacy laws.

If you have saved last year’s forms this should take only a couple of minutes, less if there are few changes. Part B includes immunizations and neither require a healthcare provider visit. Part C should be submitted at a time of your choosing, and requires an annual physical examination, with a healthcare provider stamp/signature and date. This is required for all participants on trips over 72 hours, but is highly suggested for all scouts and participating parents!

For general information about recommended vaccinations for school-age individuals or more information see the CA Dept of Public Health’s site: www.shotsforschool.org.

Vaccination also applies to adults. The current suggestions for booster vaccines each decade include the Tetanus or the combinedTDaP (or DTP), which vaccinates against Tetanus, Diptheria and Pertussis. There are others as well.
